Cortisonglaukom nach Nierentransplantation

Abstract
Bei einem 32jährigen Patienten mit chronischer Nephritis, bei dem eine Nierentransplantation vorgenommen worden war, entwickelte sich 7 Wochen nach beherrschter akuter Abstoßungsreaktion ein Cortisonglaukom beider Augen mit intraokulären Druckwerten von 80 mm Hg. Kombinierte Behandlung mit Pilocarpin und Acetazolamid führte zu weitgehendem Druckabfall innerhalb von 2 Tagen auf 30 mm Hg am rechten und 28 mm Hg am linken Auge. Die Corticosteroidmedikation konnte aus vitaler Indikation zur Abstoßungsprophylaxe nicht abgesetzt werden. A 32-year-old patient with chronic nephritis received a renal homograft. Seven weeks after control of an acute rejection episode corticosteroid-induced glaucoma developed in both eyes with intraocular pressures of 80 mm Hg. Combined treatment with pilocarpine and acetazolamide led to pressure reduction within two days to 30 mm Hg in the right and 28 mm Hg in the left eye. Steroid treatment could not be stopped due to the absolute indication of rejection prophylaxis.
