Abstract
The Family Health Care Project is an experimental attempt to answer the need for a health care system which will bridge the gap between patient needs and available but fragmented health resources. The health care team, which consists of physician, public health nurse, and health aide, coordinates the efforts of medical and allied medical personnel to give comprehensive health care to multiproblem urban families. There are difficulties in creating a coordinated health team, given the traditional role relationships of the doctors, nurses, and other health personnel. After 2 1/2 years of experience with this model, it was possible to develop an effectively functioning health team when communication techniques were improved. The data presented reveal the effect of health team delivery of care on utilization patterns of urban families.Keywords
