Temporary Unilateral Pulmonary Artery Occlusion in the Preoperative Evaluation of Patients with Bronchial Carcinoma

Abstract
In 142 patients with bronchial carcinoma a comparison was made between the results of pulmonary function studies and pressures before and during TUPAO (temporary unilateral pulmonary artery occlusion). Correlations were found between residual- and forced expiratory volume, both as a percentage of the total lung capacity, and also between the pulmonary pressures before and during TUPAO. No significant correlation existed between pulmonary function data and pulmonary artery pressures, neither between O2 uptake of the affected lung and pulmonary artery pressure before and during TUPAO. No significant differences as to the pre-operative findings were found between pneumonectomy and lobectomy patients. No correlation existed between early postoperative mortality and pre-operative function findings. Right ventricular perforation occurred in 3 cases and these complications are presented in detail. TUPAO should be reserved for very distinct circulatory indications, such as the evaluations of shunts. Suggestions for preventing such serious complications are given. The most useful tests in patients with bronchial carcinoma are spirometry, broncho-spirometry and conventional right heart catheterization.This publication has 0 references indexed in Scilit:
