Outcome of Multifetal Gestations: A Case-Control Study

Abstract
Higher order multifetal gestations are increasingly common. The advent of multifetal reduction procedures has increased the need for outcome data with which to counsel such couples. This case control study was designed to examine the outcomes of triplet or higher order gestations cared for over a ten-year period. Thirteen cases (eleven triplet, two quadruplet gestations) were matched to twenty-six twin controls for parity, maternal age, and delivery service. Outcomes were determined by chart review using standard definitions. Short-term perinatal mortality and morbidity were similar between the two groups. Achieving these similar outcomes necessitated a high rate of hospital admission for mothers, almost universal need for intensive care nursery admission for infants, and a frequent use of abdominal delivery, often via vertical uterine incisions. This information may benefit clinicians counselling couples with higher order multifetal gestations.
