Abstract
Summary The rare incidence of malignant change in pilonidal disease cannot be explained. A squamous cell epithelioma was detected by biopsy in the case reported herewith. The operation of wide excision and primary closure was apparently successful in this case when other methods failed, but this does not indicate the advantage of one procedure over another. The previous surgery had been done in a different era of pilonidal surgery. The wound healed by first intention and, of even more importance, it has remained healed beyond the arbitrary five-year period.
