In 1966 Asai described an operation method for the restoration of the voice, following a total laryngectomy, by means of a plastic construction of a prepharyngeal air passage between trachea and oral cavity. With regard to my own experiences made in 7 cases, a thorough investigation of the indication is postulated. The indication for the operation is described in detail and, at the same time, a simplifying modification of the procedure after the approved pharynx plasty by Gluck and Soerensen is presented. The disadvantages of this method (necessity of permanent use of cannulae, possibility of saliva and food particles entering into the lower air passages, the relatively long interval between the treatment of the main disease and the ray therapy which usually is necessary later) should save us from transforming a possibility beneficial in appropriate cases into a rule which, in other cases, is contrary to the basic principles of the treatment of carcinoma.
