Abstract
A new system of catheters for coronary dilation is described. Its special feature is that dilation catheters with different balloon sizes, or other catheters for intracoronary instrumentation can be interchanged via a guide wire located transstenotically. In particular, arterialized blood can be transfused into the distal coronary segment by means of a special transfusion catheter in the presence of coronary occlusion. This is achieved with the aid of a tube roughly 3 cm long, at the distal end of the dilation catheter, which accomodates the guide wire, which is otherwise located within the introducer catheter parallel alongside the dilation catheter shaft. In this way, the dilation catheter can be moved backwards and forwards over the guide wire as on a sliding rail. Further advantages are improved controllability of the guide wire which can be moved largely free of friction, and the improved flow of contrast medium via the introducer catheter containing the dilation catheter and the guide wire, through a reduction in the size of the dilation catheter shaft. A higher success rate and greater safety of coronary dilation owing to the more reliable application of transfusion catheters are to be expected, with a consequent lowering of the mortality risk. Organizational simplifications with regard to the acute surgical standby generally necessary in dilation are conceivable. Moreover, simplified handling implies a reduction of investigation time and the radiation dose.
