Sexual problems in arthritis and allied conditions

Abstract
Summary There is need for a greater readiness among the medical and paramedical professions to recognize the widespread occurrence of sexual problems in patients with arthritis and allied conditions. Sexual image, sexual drive, sexual competence, sexual expression, and sexual opportunity may all be adversely affected by the onset of arthritis, the type and degree of effect depending on the nature, severity, and duration of the disease and on the social and marital state of the patient. Single patients may have difficulty in forming stable sexual relationships and married patients in creating a mutually satisfying pattern of sexual expression. The physical and psychological state of patient and partner and the medication of both need careful consideration to determine contributory causes to the individual problem-complex of each patient. Based on the experience of 12 years' sexual counselling at a disabled living unit and an orthopaedic hospital, and of 4 years at a weekly clinic for disabled people with sexual problems, the causes and means of amelioration of these problems are discussed. Résumé Il est nécessaire que les professions médicales et paramédicales prennent conscience de la fréquence des problèmes sexuels chez les patients atteints d'arthrite et de conditions similaires. L'image de soi, la libido, la compétence sexuelle, l'expression sexuelle et les occasions de contacts sexuels peuvent tous être modifiés dès le début de l'arthrite, le type et le degré de l'effet dépendant de la nature, de la sévérité et de la durée de la maladie, ainsi que du statut social et marital du patient. Les patients non mariés ont souvent de la peine à former des relations sexuelles stables, et les patients mariés à établir un état d'expression sexuelle mutuellement satisfaisant. Zusammenfassung Es besteht das Bedürfnis nach grösserer Bereitschaft der Aerzte und ihrer medizinischen Mitarbeiter, die Häufigkeit sexueller Probleme bei Patienten mit Arthritis und anderen Gelenks- und Bindegewebs-krankheiten zu erkennen. Sexualbewusstsein, sexuelle Aktivität, Kompetenz, Ausdrucksfähigkeit und Gelegenheit können gestört werden durch den Beginn einer Arthritis. Art und Schweregrad der Störung sind sowohl abhängig von der Art, Schwere und Dauer der Krankheit wie von den sozialen und ehelichen Verhältnissen des Patienten. Unverheiratete habenoft Schwierigkeiten, dauerhafte sexuelle Beziehungen herzustellen. Für verheiratete Patienten ist es oft schwierig, ein beidseitig befriedigendes Sexualleben zu realisieren. Der physische und psychische Zustand des Patienten und seines Partners müssen ebenso sorgfältig beachtet werden wie die Verordnung von Medikamenten, welche unter Umständen die bestehenden Schwierigkeiten vergrössern können. Die Ursachen der entsprechenden Probleme und die Möglichkeiten ihrer Lösung werden auf Grund der 12-jährigen Erfahrungen in der Sexualberatung in einem Behindertenheim und an einer orthopädischen Klinik und einer vierjährigen Erfahrung in einer wöchentlichen ambulanten Sprechstunde für Behinderte mit sexuellen Problemen diskutiert.
