STUDIES OF URINARY CORTICOSTEROID BY A METHOD PERMITTING ANALYSIS OF STEROIDS POORLY SOLUBLE IN WATER

Abstract
Eleven patients with adrenal insufficiency were studied by a method permitting measurement of "total corticosteroid," of "water-soluble corticosteroid," and of "corticosteroid poorly soluble in water." The corticosteroid excretion of these subjects was low during the time when severe insufficiency was present, but during the period without treatment before signs of insufficiency developed and during treatment some of these patients excreted considerable amts. of corticosteroid. When these patients received moderate amts. of desoxycorticosterone acetate resulting in good clinical control, the corticosteroid excretion increased towards or reached the lower range of normal. This corticosteroid was water soluble, indicating that conversion of desoxycorticosterone to a different compound had occurred. When an excess of desoxycorticosterone acetate was given, a large amt. of corticosteroid was excreted, most of which was poorly water soluble. A much larger fraction of the adrenal steroid administered was recovered as corticosteroid after the admn. of large amts. of desoxycorticosterone or whole adrenal cortical extract than when smaller doses were given.
