Abstract
This article examines how therapeutic decisions by occupational therapists are shaped by the impact of a framework of functional independence and the organization of rehabilitation services. Insights are presented from an exploratory pilot study using focus group methodology. Analysis of the focus groups revealed three therapeutic dilemmas or paradoxical situations that occur in caring for an older patient. The therapeutic dilemmas raised ethical issues regarding an older adult's right to autonomy and decisional control throughout the rehabilitation process and the ability of therapists to develop context-based treatment for the elderly. Keywords
