Tibial Periosteal Graft in Repair of Cleft Lip and Palate

Abstract
Present surgical repair of the anatomoembryological primary palate must respect the palatal mucoperiosteum, where wide dissection would cause scar retraction and maxillary deformities, and respect the width and function of the nostrils. For these reasons, the authors, following the technique proposed by Stricker and Chancholle, used a free graft of tibial periosteum to repair the bony alveolopalatal cleft during a primary labial operation. Our aims were to reconstruct the oral layer while leaving the palatal mucoperiosteum free of scars, to ensure a dynamic bony production along the whole maxillary cleft and under the alar base of the nose, and to transform in a single step a total cleft to a simple cleft of the soft palate. The authors comment on the preliminary results of their six-year experience with this technique.
