Abstract
The in vitro assay of serum bactericidal activity in several recent patients with severe infection has shown an unexpected prozone effect. That is, the undiluted, or minimally diluted serum from a patient receiving antibiotic therapy with cephalothin showed growth of a microbial inoculum, whereas more dilute specimens inhibited growth. The effect was sufficiently extensive to lead to confusion in clinical interpretation. Replication of this effect with human serum albumin plus stock organisms and antibiotic has led to a possible explanation of this phenomenon based on differential protein binding. Keywords
