Wegener's Granulomatosis Following Stapedectomy

Abstract

THE formation of granulomas in the middle ear as a complication to stapedectomy is relatively uncommon. Some cases have been published by Harris and Weiss¹ and by Rock.² Kaufmann and Schuknecht³ have recently (1967) described ten more cases. In all of them the granulomas had a benign histological picture and appeared mostly within the first six postoperative weeks. Absorbable gelatin sponge (Gelfoam) wire implants resulted in such reparative granulomas as often as fat-wire implants. The main symptom was curiously enough a bone conduction hearing loss with little or no air conductive component. After reoperation and cleansing of the oval window the hearing was reestablished. As far as we know, no case of Wegener's granulomatosis has been reported in the middle ear following stapedectomy.
