SPLENOMEGALY IN CONGESTIVE HEART FAILURE

Abstract
The avg. wt. of the spleen at autopsy was detd. in 100 cases of congestive heart failure, half of whom had rheumatic valvular heart disease, and half having non-valvular heart disease. The avg. wt. in the valvular group was 210 g.; in the non-valvular group, 198 g. In 50 controls, the avg. spleen wt. was 135 g. In the rheumatic group, there were 20 definitely enlarged spleens (over 200 g.) and there were 19 definitely enlarged in the non-valvular group. 13 of 97 patients with congestive failure had spleens in the range of theoretical palpability (over 300 g.). Only 18 of 97 patients with congestive heart failure had definitely enlarged livers. The wt. of the avg. liver was not increased. Duration of the congestive failure or the presence of splenic infarcts did not influence the wt. of the spleen. 11 cases of cardiac cirrhosis were found in the patients with valvular heart disease. These had all been in congestive failure for over 6 mo.; here the avg. spleen wt. was 274 g. Only one case of cardiac cirrhosis was found in the non-valvular grouo.Keywords
