Three patients with profound renal failure had only minimal dilatation of the collecting systems on initial evaluation with ultrasound and computed tomography. All had obstructive nephropathy that was reversed by percutaneous drainage. When no obstruction is documented by these screening modalities and the kidneys are normal in size or enlarged, further evaluation is necessary to exclude obstruction.
