Giant condylomata acuminata (Buschke-Lowenstein tumor) of the penis resembles carcinoma in some respects but has a favorable prognosis. The authors reporting the 50-odd cases in the literature emphasize the occurrence in uncircumcised males with phimosis. All observed rapid growth, benign histology, failure of conservative measures, and all advised surgical removal. An additional case presenting similar findings and requiring surgical treatment after conservative therapy failed is presented in some detail. Immediate circumcision should be carried out in all males with phimotic foreskins who develop verruca acuminata in this area in order to prevent the development of this lesion. A seven-year follow-up, with no evidence of recurrence, exemplifies the necessity of differentiating this tumor from squamous cell carcinoma.
