Abstract
Much of the emergency pressures faced by acute trusts result from patients with acute exacerbations of chronic illness. Progress can be made if greater emphasis is placed on managing the patient and their disease throughout the life-cycle. A new model of care was developed termed 'comprehensive care co-ordination'.
