Thumb web contracture

Abstract
Thumb web space release in the hand is designed to achieve maximum return of normal function of the thumb. This requires careful thought on the part of the hand surgeon in the design of incisions and providing for skin cover. Fibrosed and shortened muscles must be released along their origin or excised. As much function as possible should be retained. Tight joint ligaments and capsule must be releasedso that the optimal angle (40 degrees) is obtained between metacarpals of thumb and index finger. Motors must be provided if the muscle power in the thumb is unbalanced. This usually requires a secondary procedure and tendon transfers. Retention in the released position is absolutely essential for several weeks postoperatively. Motion is initiated as early as wound healing will permit.
