Treatment of Constitutional Delay of Growth and Adolescence with Human Growth Hormone

Abstract
Constitutional delay of growth and adolescence is caused by continuous diminution of spontaneous hGH-secretion as was proved by measuring hGH-levels in deep sleep by night. Therefore, treatment by hGH is a replacement therapy. Thirteen patients underwent hGH long-term therapy, 12 with good success. On average we observed doubling of the previous growth velocity, in the most favourable case an increase from 3.5 cm to 14.7 cm per year. Predicted adult height rose in 5 cases. Die Ursache der konstitutionellen Verzögerung von Wachstum und Adoleszenz ist eine gegenüber normalen Kontrollen kontinuierlich verminderte Spotansekretion von Wachstumshormon (WH), die durch Messung der WH-Spiegel im nächtlichen Tiefschlaf gut erfaßbar ist. Die Therapie mit WH ist dementsprechend eine Substitutionsbehandlung. Dreizehn Patienten wurden langfristig mit WH, 12 I.E./ Woche, behandelt, 12 davon mit gutem Erfolg. Im Mittel resultierte eine Verdoppelung der Wachstumsgeschwindigkeit, im besten Fall ein Anstieg von 3,5 auf 14,7 cm pro Jahr. In 5 Fällen nahm die vorausgesagte Erwachsenengröße zu.
