Abstract
In a controlled clinical trial, 226 patients received 1 g cephaloridine and 221 patients 0.5 g framycetin into the wound before skin closure. In clean operations primary sepsis occurred once with each antibiotic but in those which were potentially con taminated cephaloridine was significantly more effective than framycetin. The secondary wound sepsis rate was unaffected by the use of an antibiotic during operation When contamination was probable during surgery (as revealed by bacteriological study) the primary wound sepsis rate was significantly higher than in those cases in which the swab taken at operation was sterile In five trials with cephaloridine, involving 2491 operations, the primary wound sepsis rate with cephaloridine was less than half the control rate and the difference was significant whatever the degree of contamination
