Abstract
An investigation into adverse reactions to drugs arising in a hospital over a 5-wk period is described, and the role of the ward pharmacist in detecting and reporting these reactions is discussed. Sources of information available to confirm a diagnosis of adverse drug reaction are described and the problem of establishing clinical significance is examined.
