The narcoleptic tetrad with spontaneous diplopia and strabismus

Abstract
A 41-year-old male was presented showing the complete tetrad of narcolepsy which began at the age of 19. In addition, the patient also experiences diplopia, uncommon as a spontaneous, isolated event in the narcoleptic syndrome. Transient, divergent strabismus accompanying the diplopia is demonstrated by photograph. The incidence of various symptoms of the syndrome is discussed as well as the relation of the symptoms to normal hypnagogic and hypnopompic phenomena. The importance of emotional factors is discussed and a psychophysiologic etiologic mechanism is considered.
