Abstract
A case of acute gangrenous appendicitis in the sac of right inguinal hernia in a 77 year-old man is described. This condition is extremely uncommon; it is very difficult to establish a correct diagnosis preoperatively; a high mortality rate is due to extensive peritoneal contamination; primary hernia repair is indicated.Keywords
