Long-term results of marmor arthroplasty in varus osteoarthritic knees

Abstract
Twenty-one osteoarthritic knees with an average varus angulation of 13° were followed up for 7–10 years after resurfacing with the Marmor compartmental knee arthroplasty. There were 14 unicompartmental and 7 bicompartmental replacements, a total of 28 implants. The knees were assessed clinically according to the Hospital for Special Surgery knee rating scale. At the latest follow-up, 19 of these cases were still excellent or good. None of the knees had been corrected by more than the tightness of the ligaments allowed, and a varus inclination of an average of 2° remained after operation and at the latest follow-up. Radiolucency of more than 2 mm around the tibial component was found in 2 out of 28 of the cases, but this was not associated with pain or disability, nor was wire breakage, found in 6 out of 28, or positional changes of the tibial component, which were seen in five patients. It is concluded that the Marmor compartmental knee arthroplasty is still indicated in osteoarthritic varus knees. Vingt et un genoux, présentant une déviation moyenne en varus de 13°, ont été suivis 7 à 10 ans après arthroplastie par prothèse compartimentale de Marmor. Il s'agissait de 14 remplacements uni-compartimentaux et de 7 bi-compartimentaux, soit un total de 28 implants. L'état des genoux a été évalué cliniquement selon la cotation de l'Hospital for Special Surgery. Au dernier examen 19 de ces cas pouvaient encore être estimés «excellents» ou «bons». Aucun de ces genoux n'avait été corrigé davantage que ne le permettait la tension ligamentaire et il persistait en moyenne 2° de varus tant après l'opération qu'au dernier examen. Il existait, 2 fois sur les 28 cas, un liseré supérieur à 2 mm autour de la pièce tibiale mais ne s'accompagnant ni de douleur, ni de gêne fonctionnelle. Il en était de même des 6 ruptures de fils métalliques et des déplacements des plateaux prothètiques observés chez 5 malades. On peut en conclure que l'arthroplastie compartimentale de Marmor conserve ses indications dans les arthroses sur genu varum.Keywords
