RESPIRATORY OBSTRUCTION IN THE NEWBORN AND EXCESS PULMONARY FLUID

Abstract
When obstruction of the bronchial tree occurs in utero, there is impaired drainage of normally contained fluid from the lung or lobe distal to the obstruction and a characteristic roentgen pattern is observed in the newborn period. There will be an increased volume of the involved lung or lobe with a uniform mass or fluid density and no air bronchogram. Whenever these findings are observed in a newborn with respiratory distress, the possibility of remediable airway obstruction must be considered. Subsequent roentgenograms may well show the characteristic obstructive emphysema usually associated with these conditions, and special studies may be necessary to define the precise anatomic defect. Two cases of respiratory obstruction in the newborn with excess pulmonary fluid are reported. One patient had congenital lobar emphysema involving the right middle lobe and the other had stenosis of the right main stem bronchus associated with anomalous origin of the left pulmonary artery.Keywords
