A pseudo-boutonniere deformity

Abstract
A pseudo-boutonnière deformity is described; it is characterised by flexion contracture of the proximal interphalangeal joint, hyperextension of the distal joint, and radiological evidence of calcification, anterior or volar to the proximal phalanx. The clinical entity appears to be due to a partial or complete rupture of the proximal attachment of the volar plate, with involvement of the oblique retinacular ligament, and the intrinsic lateral bands on one side of the finger. When the flexion contracture of the proximal interphalangeal joint is less than 40°, the deformity can be controlled by conservative means; the more severe cases require surgical reconstruction.
