Impedance cardiography, a method to evaluate quantitatively cardiac output? Comparison with the Fick principle

Abstract
In 36 children without shunts nearly 280 electrical impedance measurements were carried out in order to estimate the accuracy of the impedance cardiography as a means of calculating cardiac output. During evaluation up to 22 combinations of test conditions and possibilities of evaluation per child were tested. As a reference method theFick Principle was used. The reproducibility of impedance measurements showed itself to be high, but no agreement of the results by impedance cardiography and corresponding values by theFick method could be found. Some reasons give rise to suppose that impedance cardiography only reflects changes of the intrathoracic fluid level occurring during heart action from which because of formal reasons no inference should be drawn about the original cardiac output. Besides physical arguments tell against the possibility of picking up these volume-changes quantitatively by analyzing transthoracic electrical impedance. An insgesamt 36 Kindern ohne Shuntvitien wurden etwa 280 Impedanzmessungen vorgenommen, um die Aussagekraft der Impedanzkardiographie über die Größe von Herzzeit- bzw. Herzschlagvolumina prüfen zu können. Zur Auswertung der Impedanzkardiogramme wurden pro Kind bis zu 22 Kombinationen von Meßbedingungen und Auswerteverfahren herangezogen. Als Bezugsmethode diente dasFicksche Prinzip. Die Reproduzierbarkeit der Impedanzmessungen war sehr gut, eine Übereinstimmung zwischen den Ergebnissen der Impedanzkardiographie mit den entsprechenden Werten derFickschen Methode bestand jedoch nicht. Es gibt Gründe für die Annahme, daß die Impedanzkardiographie lediglich herzsynchrone Schwankungen des intrathorakalen Flüssigkeitspegels widerspiegelt, von denen aus formalen Gründen jedoch nicht auf das Ausgangsschlagvolumen geschlossen werden sollte. Außerdem sprechen physikalische Gründe gegen eine quantitative Erfaßbarkeit dieser Volumenschwankungen.Keywords
