Abstract
The Rural Efforts to Assist Children at Home (REACH) model of decentralized case management has been found to be an efficient and effective method for (a) coordinating and structuring the health, social service, and educational needs of complex chronically ill children and their families and (b) providing to families the training and support they need to effectively assume the role of primary case manager. The REACH administrative, training, care planning, and staff monitoring systems are presented as a model for family-centered community-based case management.Keywords
