Does the frequency of paracolostomy hernias depend on the position of the colostomy in the abdominal wall?

Abstract
The aim of this study was to determine the rate of paracolostomy hernia in relation to transrectal or pararectal position of a colostomy. Fifty-four consecutive patients who had undergone an abdomino-perineal excision for rectal neoplasia were studied. The location of the colostomy was assessed by physical examination. Computerised Tomography (CT) scanning was used in the cases where its anatomical position remained in doubt. The colostomy was in a pararectal position in 29 (54%), while in 25 (46%) it was transrectal. CT was necessary to determine the location of the colostomies in 9 cases (16%), eight of which had a parastomal hernia. The colostomy was pararectal in 15 (52%) of the 26 patients who had a paracolostomy hernia, and in 14 (48%) of the 28 patients without a paracolostomy hernia. No statistically significant correlation was found between the presence of a parastomal hernia and the position of the stoma in the abdominal wall. Cette étude a pour but de déterminer l'incidence de hernies parastomiales selon que la colostomie soit placée en position transrectale ou pararectale. Cinquante quatre patients consécutifs ayant subi une amputation abdomino-périnéale pour cancer rectal ont été étudiés. La position de la colostomie a été précisée par l'examen clinique. En cas de doute, une tomodensitométrie (CT) a été pratiquée. La colostomie était en position pararectale 29 fois (54%) et en position transrectale 25 fois (46%). Le CT s'est révélé nécessaire chez 9 patients (16%) pour déterminer la position de la stomie; 8 de ces patients, soit 16% des 54 malades, présentaient une hernie parastomiale. La colostomie était pararectale chez 15 des 26 patients (52%) qui présentaient une hernie parastomiale, de même que chez 14 des 28 patients (48%) qui ne présentaient pas de hernie. Aucune différence statistiquenment significative n'a été observée entre la présence d'une hernie parastomiale et le trajet de la stomie au travers de la paroid abdominale.Keywords
