SPLENIC RUPTURE IN INFECTIOUS MONONUCLEOSIS

Abstract
Two cases of splenic rupture occurred in young soldiers with mononucleosis. Both recovered after splenectomy. The clinical picture of uncomplicated mononucleosis is not protean. Severe abdominal pain is rare. Therefore, pain in the left upper abdomen, especially if it radiates to the left shoulder, suggests splenic rupture. Such pain is even more significant if accompanied by an increasing pulse rate; and shock and evidence of acute anemia make diagnosis obvious. Early operation may be life saving. Microphotographs, revealing striking infiltration and weakening of capsule of spleen by mononuclear cells, explained mechanism of rupture.
