Interictal Spike Localisation with Methohexitone: Preoperative Activation and Surgical Follow-Up

Abstract
Summary: Thirty‐four patients with intractable temporal lobe epilepsy underwent preoperative and perioperative EEG activation with methohexitone sodium. Subse‐ quently, all patients underwent anterior temporal lobec‐ tomy with deep structure preservation. Preoperative spike localisation with methohexitone was concordant with peroperative recording in most cases. Large doses of methohexitone were administered to two patients, rendering the EEG almost isoelectric. Areas of abnormal spiking activity were refractory to the effects of metho‐ hexitone, and recovery was marked by an increase in spike frequency. Postoperative clinical evaluation con‐ firmed the specificity of methohexitone for abnormally epileptogenic tissue. RÉSUMÉ 34 patients présentant une épilepsie temporale rebelle ont bénéficié d'une activation de l'EEG au moyen de la methohexi‐tone sodique. Tous les patients ont subi ensuite une lobectomie temporale antérieure respectant les structures profondes. La localisation pré‐opératoire des pointes au moyen de la methohexi‐tone concordait dans la plupart des cas avec l'enregistrement pré‐opératoire. De fortes doses de methohexitone ont été ad‐ministrées à 2 patients, elles ont abouti à un EEG presque isoélectrique. Les zones d'activité irritative anormale se sont montrées réfractaires aux effets de la methohexitone, et la récupération a été marquée par une augmentation de fréquence des paroxysmes. L'évaluation clinique post‐operaloire a confirme la specificite de la methohexitone sur le tissu épileptogénique pathologique.Keywords
