VARIATIONS IN RENAL EXCRETION OF SODIUM INDEPENDENT OF CHANGE IN ADRENOCORTICAL HORMONE DOSAGE IN PATIENTS WITH ADDISON'S DISEASE*

Abstract
Three patients with Addison''s disease maintained on a constant dose of cortisone and desoxycorticosterone acetate (DCA) were subjected to stimuli which are known to induce prompt changes in renal excretion of Na in normal man. Appropriate changes in natriuresis were observed in relation to change in posture, infusion of hypotonic solution of sodium chloride-bicarbonate mixture, venous congestion of the thighs, and diurnal rhythm. Changes in Na excretion were associated with alteration in tubular re-absorption rather than filtered load. Cortisone appeared necessary to maintain integrity of the renal regulatory mechanism in these patients but changes in Na excretion were presumably independent of change in hormone activity.Keywords
