Presentation of Case First admission. A fifty-year-old chauffeur entered the hospital because of pain in the side.Five weeks previously, after a heavy intake of alcohol, the patient "had a pretty hard fall," but he could not recall details of the injury. One week later he noticed pleuritic pain in the lower portion of the left side of the chest that radiated to the periumbilical region, the neck and the left shoulder.
