Management of Cold Urticaria during Hypothermic Cardiopulmonary Bypass

Abstract
Cold urticaria is a potentially life-threatening allergic disorder characterized by swelling and edema of exposed tissue in response to a cold stimulus, and occasionally associated with hypotension and cardiovascular collapse.1 2 3 This report presents our experience with the cold-urticaria syndrome in a patient who required myocardial revascularization using hypothermic cardiopulmonary bypass.MethodsA 47-year-old man with a history of two myocardial infarctions presented with crescendo angina. He was receiving propranolol, 20 mg four times daily. He had a six-year history of localized swelling and pruritus after cold exposure, most noticeable after eating ice cream or holding a cold drink.
