Abstract
Presentation of Case First admission. A 62-year-old man was admitted to the hospital because of pain in the chest.He was well until four years earlier, when exertional angina pectoris developed, and hypertension was found. During the two years before admission he was dyspneic on exertion; angina occurred daily and increased in severity. Isosorbide dinitrate, propranolol, and hydrochlorothiazide were prescribed, with uncertain relief. On the day of entry chest pain recurred and persisted for several hours.The patient had smoked three packages of cigarettes daily in the past and drank eight cans of beer per day, with occasional drinks . . .
