A Multidisciplinary Approach to Case Management of Frail, Hospitalized Older Adults

Abstract
Case management has been operationally defined in many ways, depending on the model and the needs of the population. This article describes a multidisciplinary resource utilization model of case management in a geriatric acute care medical unit that differs from many nursing- and protocol-driven case management models. The author discusses the design, implementation, and outcomes, which include improved quality of care, decreased length of stay, improved financial results, and increased physician compliance.
