Abstract
Our rapidly changing health care reimbursement system demands that nurses, therapists, social workers, and other team members increase their knowledge of documentation and how it affects reimbursement, patient care, and legal liability. This article is intended to increase the professional caregiver's awareness of the effects of charting on both the home health client and the home health agency. It reviews the advantages of comprehensive documentation and provides practical advice on how to avoid common charting pitfalls. It also urges the establishment of a link among the major components of the chart.
