Abstract
In this paper a discussion of some problems of ultrasonic diagnosis of bile stones and a critical evaluation of the A-scanning method have been presented. On the basis of the experimental results the author suggests several modifications of the A-scanning method which he calls the impulse reflection passage ultrasonic method using two transducers. Experimental measurements were carried out on tissue specimens from 60 healthy subjects, 70 patients with abnormal affected liver parenchyma and 946 patients suffering from cholelithiasis. The results were compared with the X-ray findings and with those obtained at surgery and on section. In cases of tumours, abscesses, severe cirrhosis, and adipose gallbladder wall, echoes may arise which cannot always be differentiated from those of bile stones. The first X-ray exploration of patients with bile stones gave positive findings in 82.87% while the ultrasonic method recorded echoes from bile stones in 85.89%. Attention is called to the fact that in cases of striking reflections there may be present tissue localities of ordinary properties which need not always be bile stones. There occur even such cases where the wave impedance of bile stones does not show any marked difference from that of surrounding tissues, which prevents detection of such bile stones. Keywords
