Abstract
Die operative Knochenbruchbehandlung ist auch in der Nachbehandlung eine verantwortungsvolle Aufgabe. Sie ist wesentlicher Bestandteil und erlaubt die Operation einer geschlossenen Fraktur zu verantworten. Schematisch läßt sich die Nachbehandlung in drei festgelegte Zeiträume einteilen: die primäre postoperative Phase, die Phase der vollständigen Entlastung und die Phase der Teil- und Vollbelastung. Gerade die zweite Phase der vollständigen Entlastung stellt an Patient und Arzt erhebliche Ansprüche. Richtlinien für die Aufnahme einer Teil- oder Vollbelastung können nur schematisch gegeben und müssen je nach Patient, Frakturtyp und Wahl der Osteosynthesemittels modifiziert werden. Besondere Beachtung verdient der klinische und röntgenologische Verlauf. The management following operative fixation of fractures by osteosynthesis is an essential part of the surgical procedure. The postoperative treatment is divided into three phases:- (1) primary postoperative period; (2) period of complete relief of weight-bearing and active movement; and (3) phase of partial and finally full activity. It is especially the second phase which makes considerable demands both on patient and doctor. Only schematic rules for resumption of partial or total load-bearing and activity can be given: they must be modified according to patient, type of fracture and method of osteosynthesis. The clinical and radiological course requires particular attention. Acerca del tratamiento subsiguiente a la ósteosíntesis El tratamiento quirúrgico de las fracturas óseas es una misión llena de responsabilidad, incluso en el tratamiento posterior. El es parte integrante esencial y permite responsabilizar la operación de una fractura cerrada. Esquemáticamente se puede dividir el tratamiento posterior en tres lapsos de tiempo fijos: la fase postoperatoria primaria, la fase de la descarga completa y la fase de la carga parcial y total. Precisamente la segunda fase, la de la descarga completa, plantea al paciente y al médico notables exigencias. Normas para la admisión de una carga parcial o total sólo se pueden dar esquemáticamente y tienen que modificarse de acuerdo a cada paciente, tipo de fractura y elección del medio de ósteosíntesis. Especial consideración merece la evolución clínica y radiológica.
