Correlation of Serum Prolactin and LH Response to Stimulation with LH-RH Before and After Extirpation of Pituitary Adenomas with Clinical Outcome

Abstract
Serum levels of prolactin and LH in response to an iv injection of 50 μg LH-RH were determined by RIA before (30 days) and after (between 30 to 45 days) surgery of pituitary adenomas in seventeen women (age 18 to 37 years) who had presented with long standing amenorrhea. Histologic observation confirmed this diagnosis. Each patient was followed without medication for one control year in order to detect spontaneous recovery of regular menses. The possible correlation between prolactin and LH levels (basally and after LH-RH) before and after surgery and the clinical evolution of the patients was examined. LH basal levels before and after surgery did not exhibit important modifications and were closely similar in both groups. Average LH response to LH-RH increased significantly after surgery in patients who recovered regular menses and showed no significant change in patients who did not recover regular menses. Differences between groups were significant after surgery. Average basal prolactin levels decreased after the operation in both groups, but in three patients who recovered regular menses and in the majority of patients who did not recover them levels were still higher than the normal 30 to 45 days after surgery. These results suggest a different pituitary secretory reserve for LH in each group after surgery, a fact that could be of some practical value in the prognosis of these patients.
