Misinterpretation of Elevated Blood Diastase Due to Impaired Renal Clearance

Abstract
Introduction It has been previously pointed out that the blood diastase values may be elevated in patients with diseases other than acute pancreatitis.* Somogyi1 considered the normal range of blood diastase to be between 60 and 200 units per 100 ml. of serum. However, in a study of the serum amylase of all admissions to a general hospital. Heifetz, Sicher, and Sachar found that about 10% of the patients had blood diastase values above 200 at the time of their entry into the hospital. This is shown in Figure 1, which depicts the blood diastase in 2000 consecutive patients at the time of their admission to the Jewish Hospital of St. Louis.† Clinical evaluation of the number of patients with a blood diastase of more than 200 indicated that less than 10 of them had a clinical picture consistent with pancreatitis or parotitis.
