Recurrent pulmonary embolism after inferior vena caval interruption with a greenfield filter

Abstract
Twenty-six patients have undergone inferior vena caval interruption with a Greenfield filter. One patient (3.8%) developed a fatal, recurrent pulmonary embolism. Postmortem examination documented the presence of thrombus within the filter, proximal extension of thrombus beyond the filter apex, and fresh pulmonary emboli. In a combined series of 556 patients, 12 patients (2.2%) developed recurrent pulmonary emboli, 2 (0.36%) of which were fatal. Including case reports, 18 patients have had documented recurrent pulmonary emboli following Greenfield filter insertion. In 4 patients, the recurrent embolism was fatal. Six (60%) of 10 patients had a concomitant malignancy. A work-up and treatment plan for patients having recurrent pulmonary emboli after the insertion of a Greenfield vena caval filter is proposed. L'examen post-mortem a mis en évidence un thrombus au niveau du filtre, une extension de la thrombose au-delà de l'apex du filtre, et des emboles pulmonaires frais. En combinant les résultats de cette étude avec d'autres séries comportant une récidive, on retrouve 12 (2.2%) des 556 patients ayant eu une embolie pulmonaire récidivante, dont 2 (0.36%) fatales. Si l'on inclut des cas cliniques, 18 patients au total ont eu une embolie pulmonaire récidivante prouvée après insertion d'un filtre de Greenfield, dont 4 fatales. Parmi les patients ayant récidivé leur embolie, 6 (60%) des 10 patients, pour lesquels on avait un diagnostic, avaient une maladie maligne. Le bilan et la stratégie thérapeutique des patients récidivant leur embolie après insertion d'un filtre de Greenfield sont proposés. Al incluir reportes de casos individuals, se encuentra que 18 pacientes han presentado embolismo pulmonar recurrente documentado después de la inserción del filtro de Greenfield; en 4 el embolismo recurrente resultó fatal.
