The Syndrome of Irreversible Lithium-Effectuated Neurotoxicity (SILENT)

Abstract
Though the acute complications of lithium toxicity involving the central nervous system have been known for more than 70 years, it is only recently that the long-lasting sequelae of lithium intoxication have come to be discussed at length; about fifty-five cases have been reported so far. The acronym SILENT (Syndrome of Irreversible Lithium-Effectuated Neurotoxicity) has been coined recently to denote these sequelae. The present report describes the typical profile of SILENT (persisting cerebellar dysfunction) and suggests measures to decrease the incidence of this potentially serious condition, for which no definitive treatment is available. Obwohl die akuten Komplikationen der Lithiumtoxizität, die das ZNS in Mitleidenschaft ziehen, seit mehr als 70 Jahren bekannt sind, sind die Langzeitfolgen der Lithiumeinnahme erst seit kurzem in ausführlicher Diskussion behandelt worden. Bisher sind 55 derartige Fälle bekanntgeworden. Die Bezeichnung ,,SILENT" als Akronym für "Syndrome of Irreversible Lithium-Effectuated Neurotoxicity" wurde kürzlich zur Kennzeichnung dieser Spätfolgen geschaffen. Der Verfasser beschreibt einen Fall mit typischem SILENT-Profil (persistierender zerebellärer Dysfunktion) und schlägt Maßnahmen vor, wie das Auftreten dieser potentiell ernsten Erkrankung verhindert werden könnte, für welche bisher keine wirksame Therapie bekannt ist.
