Abstract
A health promotion perspective and the involvement of voluntary agencies are both becoming increasingly important in the development of services for the chronically ill. A multi-disciplinary model of health promotion for this population is presented that highlights the importance of self-care education and the contributions of social workers, health educators, and other community health professionals in the development and delivery of such a program.Keywords
