Abstract
A RECENT Institute of Medicine (IOM) report has suggested that as many as 98 000 patients die annually in the United States as a result of medical error.1 This report has been criticized because of its reliance on retrospective review of medical records by outside reviewers several years after the hospital admissions.2-4 Some5,6 have suggested that the report significantly overestimates the rate of medical error. Most literature reporting complication rates does not differentiate complications due to medical error from those due to patient disease.Keywords
