Splenic Hemopoiesis in Idiopathic Thrombocytopenic Purpura

Abstract
Splenic hemopoiesis was observed in 48 of 50 spleens from patients with idiopathic thrombocytopenic purpura who underwent splenectomy. For comparison, spleens from normal subjects (incidental spelenectomy) and patients with a variety of hematologic disorders were also studied. In nine patients with idiopathic thrombocytopenic purpura, hemopoietic tissue in the spleen was quantitatively comparable to that of patients with myeloid metaplasia with myelofibrosis. However, in idiopathic thrombocytopenic purpura, the hemopoietic tissue was limited to the extravascular space of the splenic cords, whereas in myeloid metaplasia with myelofibrosis, hemopoiesis was equally distributed between cords and sinuses of the red pulp. Awareness of this picture is essential in the evaluation of the spleen in idiopathic thrombocytopenic purpura to prevent misdiagnosis of myeloproliferative disorders.Keywords
