Abstract
A series of 111 cases of endometrial adenocarcinoma seen over a 10-year period is reviewed. A modified histologic classification, the theories of histogenesis, and the clinical biologic behavior are presented and discussed. The proposed new classification includes the usual Grades I to III (76.6%) endometrial adenocarcinomas and Group IV (23.4%) morphologically variant types of endometrial carcinomas. The latter consist of 12 cases (10.8%) of adenoacanthoma, five cases (4.5%) of clear-cell adenocarcinoma, five cases (4.5%) of embryonal anaplastic adenocarcinoma, two cases (1.8%) of mesonephric adenocarcinoma, and two cases (1.8%) of adenosquamous carcinoma. The 5-yr. survival rate of Grade I endometrial adenocarcinoma is 72.5%; Grade II, 45.4%; and Grade III, 28.7%. The 5-yr. survival rate for the Group IV morphologically variant types of endometrial carcinomas is variable and inconclusive. The follow-up data are incomplete, but by average, the 5-yr. survival rate for the Group IV tumors is nearly 40%.
