Sudden Pulmonary Dysfunction in Prosthetic Hip Replacement Surgery: A Case Report

Abstract
In a 51‐year‐old female patient (suffering from necrosis of the femoral head), the mean pulmonary arterial pressure rose from 25 to 31 mmHg, and the mean arterial blood pressure fell from 100 to 50 mmHg 20‐25 seconds after the insertion of a femoral prosthesis into the medullary cavity of the femur by the Charnley technique. Airway resistance increased from 9.0 to 12.3 dyn.s.cm‐5at an airflow of 0.5 l/s. Venous admixture during air‐ventilation increased from 17 to 52%, causing an arterial desaturation from 95 to 74% oxygen saturation.These changes indicate the presence of peripheral vasodilatation, increased pulmonary vascular resistance and constriction of the bronchioli. The possible explanations for these changes are discussed.ZUSAMMENFASSUNG: Bei einer 51‐jährigen Patientin (an einer Nekrose des Femurkopfes leidend) stieg der mittlere Pulmonalarteriendruck von 25 auf 31 mm Hg und fiel der mittlere artérielle Blutdruck von 100 auf 50 mm Hg 20–25 Sekunden nach dem Einsetzen einer Hüftkopfprothese in den Markraum des Femurs nach der Technik von Charnley. Die venöse Beimischung unter Luftbeatmung stieg von 17 auf 52 % an, eine arterielle Desaturation von 95 auf 74 % Sauerstoffsättigung verursachend. Der Atemwegswiderstand erhöhte sich von 9,0 auf 12,3 dyn/sec/cm‐5bei einem Luftstrom von 0,5 1/sec.Diese Veränderungen deuten auf das Vorliegen einer peripheren Vasodilatation, erhöhten pulmonalen Gefäßwiderstand und Bronchokonstriktion hin. Mögliche Erklärungen für diese Veründerungen werden diskutiert.Keywords
