DISSECTING ANEURYSM OF THE AORTA:

Abstract
A clinical and anatomic analysis of 17 cases of acute dissecting aneurysm of the aorta occurring at the Massachusetts General Hospital over the past 10 yrs. (1937 to 1946) is presented. A correct antemortem diagnosis was made in 8 cases, in one of which a 2d and fatal attack was wrongly interpreted. The manner of onset and the severity and spread of pain are the most reliable symptoms on which to base a diagnosis of dissecting aneurysm. A history of hypertension was established in every individual of this series. Evidence of hypertensive heart disease was found in all but 3 cases, two of which showed syphilitic heart disease. The absence of characteristic ECG. changes suggesting myocardial infarction should be given much wt. in reaching the diagnosis of dissecting aneurysm in equivocal situations. In spite of the fact that a definite diagnosis is not always easily reached, even when the condition is suspected, there has been a marked increase in the antemortem recognition of dissecting aneurysm (luring the past decade.......Keywords
