Schilddrüsenwachstum und Schilddrüsenüberfunktion bei Hypophysenvorderlappeninsuffizienz

Abstract
Bei drei Patienten mit Hypophysenvorderlappeninsuffizienz kam es zu Strumawachstum und (oder) Hyperthyreose nach Operation bzw. 90Yttrium-Implantation in die Sella turcica wegen Hypophysenvorderlappenadenomen. Differentialdiagnostisch sind bei solchen TSH-unabhängigen Schilddrüsenvergrößerungen mit oder ohne Schilddrüsenüberfunktion zu berücksichtigen: maligne Schilddrüsentumoren, Thyreoiditis, Akromegalie, Morbus Basedow und autonome Schilddrüsenadenome. Three cases of anterior-pituitary insufficiency with thyroid abnormalities are reported. In one, a malignant goitre developed five years after an adenoma of the anterior pituitary had been surgically removed. In the second case, hyperthyroidism developed after 90Y implantation for acromegaly had caused anterior-pituitary insufficiency. In the third case, toxic adenoma developed after operative removal of a chromophobe anterior-pituitary adenoma. In the differential diagnosis of such cases of TSH-independent thyroidgland enlargement, with or without thyroid hyperfunction, possible causes to be considered include malignant thyroid tumour, thyroiditis, acromegaly, hyperthyroidism and autonomous thyroid adenoma. Crecimiento del tiroides e hiperfunción tiroidea en la insuficiencia del lóbulo anterior de la hipófisis En tres pacientes con insuficiencia del lóbulo anterior de la hipófisis se presentó un crecimiento del bocio e (o) hipertiroidismo después de la operación o implantación de itrio90 en la silla turca debida a adenomas del lóbulo anterior de la hipófisis. En el diagnóstico diferencial hay que tener en cuenta en tales hipertrofias tiroideas independientes de la TSH con o sin hiperfunción tiroidea: tumores malignos del tiroides, tiroiditis, acromegalia, morbo de Basedow y adenomas tiroideos autónomos.
